1. What this covers
In exchange for ORIGIN letting the athlete named on this form take part, I agree to this release. It covers everything she does with ORIGIN, including workouts, tryouts, clinics, camps, practices, scrimmages and games (the “Activities”), for 12 months from the date I sign. It is not an agreement to join a team. “ORIGIN” means ORIGIN Softball LLC and ORIGIN Promise Foundation Inc.
2. The risks
I understand that softball can cause injuries and illness. They range from minor ones to concussions, broken bones and heat illness, and in rare cases sudden cardiac arrest, permanent disability or death. Some injuries come from the ordinary negligence of ORIGIN or others. Taking part is voluntary, and I accept these risks for myself and the athlete.
3. Release and promise not to sue
For myself, the athlete and our heirs, I release ORIGIN, its coaches, staff and volunteers, the owners of the fields ORIGIN uses, and the hosts of events she plays in from every claim for injury, illness, death or property damage that arises from the Activities, including claims based on their ordinary negligence.
I promise not to sue them, for myself or on the athlete’s behalf.
This release does not cover gross negligence or willful, wanton or intentional misconduct.
4. Health and emergencies
I confirm she is healthy enough to take part, and I will tell her coach about any health condition that affects her safety. She follows her coaches’ safety instructions, and a coach may hold her out at any time. If I cannot be reached, I authorize ORIGIN’s coaches to give first aid and get her emergency medical care, and the cost is mine. If a coach suspects a concussion or another serious injury or illness, she sits out until a licensed health care provider clears her in writing. I have read the CDC’s concussion information sheet.
5. Photos and video
ORIGIN photographs, films and streams its Activities to coach its players, to let families watch and to share the club’s story. ORIGIN may use photos and video of her from the Activities, and her name, to promote ORIGIN, online, in print, in ads and in its trademark filings. It may also share posts I tag ORIGIN in. If a court order, a foster placement or a safety concern means her image must not be shared, I will tell ORIGIN before she takes part, or as soon as I know.
6. General
Georgia law applies. If part of this release cannot be enforced, the rest still applies. I agree to sign electronically, and my typed name is my signature. ORIGIN emails me a copy.
